Please print and fill out the following forms. They should be turned in to your team mom prior to the first day of practice.
2024 Absentee Form2024 Player Contract & Parental Consent Form2024 Medical History FormYou will also need to provide (in Fall):
1. Birth Certificate - A copy will be fine. No originals this year.
2. Report card - End of last school year with all 4 quarters (copy)
3. Physical - You will need to go to your physician for sign-off on the medical form before you can practice. Make sure it's filled out completely with all vitals, height and weight.
If you are an adult and volunteering in any capacity (Coach, Team Mom, Team Doctor etc.) for the 2024 season you will be required to fill out a volunteers form which will be emailed to you prior to the season.
